• Own or Rent? Bring a COPY your ID, one of the following Proof of Residency documents:
• Mortgage Statement
• Current Lease
• Most recent Utility Bill (Gas, Electric, or Water bill accepted)
• Residency Affidavit Procedures: If enrolling parent lives with someone else in the district, both parties must attend Registration Day, provide ID, new *notarized* Residency Affidavit, two proofs of owner’s residency, official mail addressed to the parent with owner’s address on it
• Submit Emergency Medical Form
• Submit Peachtree Charter Agreement Form 
• Submit Media Release Form
