Please Bring:

✓ Official Birth Certificate (Government issued, Hospital Certificates are not accepted)
✓ Proof of Residency one gas, water or electric bill + one residence document with parent/guardian name, current date and address: mortgage payment bill, home contract, paycheck stub, state driver’s license/ID with photo, bank statement, homeowner’s/Renter’s insurance, section 8/HUD housing contract, HOA Bill/Statement.
✓ Parent/Legal Guardian ID, Guardianship documents if Legal Guardian
DOCUMENTS NEEDED TO SECURE YOUR PRE-K SPACE:
✓ Immunization Certificate (FORM 3231, available at your pediatrician’s office or local Health Department)
✓ NEW Eye, Ear & Dental & Nutrition Examination Certificate (FORM 3300 Rev 3/2013, available at your pediatrician’s office or local Health Department)
✓ Social Security Card or Waiver
✓ Medicaid/Proof of Income
